The Cortland County Health Department (CCHD), in collaboration with Guthrie Cortland Medical Center, launched its 2025 Community Health Assessment Survey to gather information from residents about local health needs, concerns, and priorities. The survey will be open from August 7th-September 15th, 2025, in English and Spanish, and is for all Cortland County residents ages 18 and older. The anonymous responses will guide future programs and partnerships to support a healthier community.

Here’s how to submit your response:

• Click this link https://www.surveymonkey.com/r/BXBSXF8 to visit the survey.
• Select your preferred language, English or Spanish, in the first question.
• Answer the survey questions– it should take about 10 minutes to complete.

For those who have difficulty accessing the online version, paper copies are available upon request.
